CFS and GMP Request Form

Use this form to submit requests to the Montana Department of Public Health for Certificates of Free Sale (CFS) and Good Manufacturing Practices (GMP) documents. These documents are often required to export food products to other countries.

In order to receive these documents, your business must already hold a food manufacturing license in the state of Montana. Your most recent inspection from your local health department or the FDA must also be free from any major violations or legal action.
